The Medical Assistant Health Coach is responsible for facilitating a seamless health and wellness experience for our members. The primary responsibilities of the Medical Assistant Health Coach center around establishing trusting supportive collaborative relationships with BlueCross BlueShield of SC (BCBSSC) and BlueChoice members; building relationships with members to navigate and support the member’s health management programs and working alongside those involved in the member’s care across all settings and services. The Health Coach Medical Assistant is committed to improving the health of our members and proactively communicates with the care team and partners to provide step-by-step guidance for members. Individuals in this role are motivated to provide exceptional service to each member they encounter and are highly driven to meet the unique needs of various members.
Location:
This role is eligible to work remotely but must be physically located within the state of South Carolina preferably Columbia South Carolina.
Work Environment:
The work hours are Monday-Friday 8:30 AM – 5:00 PM.
New Hire training will be on-site at our Percival Road Building in Columbia SC.
Description
What You’ll Do:

Provide health coaching or intervention telephonically or in-person for assigned health programs. Utilize all data systems containing patient information and document information appropriately.

Assist with the identification of areas for which there is a need for health management programs. Monitors quality indicators and develop programs to impact areas where improvement is warranted positively. Develop patient or provider educational materials.

May conduct research and analysis of reports (monthly quarterly and annually).

Establish and maintains effective communications with the provider network group representatives and members.

Associate Degree in Medical Assisting or successful completion of the postsecondary Medical Assistant Program.

Certified Medical Assistant through the American Association of Medical Assistants (AAMA) or Registered Medical Assistant through American Medical Technologists (AMA) or Nationally Registered Certified Medical Assistant (NRCMA).

Three years of clinical experience.

Ability to work independently prioritize effectively and make sound decisions.

Good judgment skills.

Demonstrate customer service and organizational skills.

Demonstrate proficiency in spelling punctuation and grammar skills.

Ability to persuade negotiate or influence others.

Analytical or critical thinking skills.

Ability to handle confidential or sensitive information with discretion.

Proficient in Microsoft Office
