Daytime positions mainly with a block of hours (3hrs) to be done in the evening or on Saturday morning according to a rotation schedule)
Description of the environment
Center médical Ste-Anne (CMSA) is a family health services organization (FHO) seeking a Nurse Practitioner (NP) who can provide care to its patient members in order to improve and/or maintain their physical and mental health and well-being. The NP will work closely with the professionals in the various programs of the Center.
Our team currently consists of 4 permanent family physicians (we are aiming for a total of 6 permanent positions by April 2023) a family physician specializing in oncology a consulting psychiatrist a nurse practitioner 2 nurses (RNs) a psychologist 2 social workers specializing in social intervention and psychotherapy a dietitian a contract orthotist-prosthetist a contract foot care nurse and 5 administrative support members. We are also partnered with another clinic consisting of 5 general surgeons on site which allows for easy access to services.
Our clinical activities are focused on 5 clinical programs: a mental health program a maternal and child care program a chronic care program (diabetes hypertension oncology) a nutrition program and a foot care program.
Our electronic medical records (EMR) are supported by Telus PS Suite software and its OCEAN add-on modules (secure messaging appointment reminder and self-booking) and videoconferencing via PS Suite/Telus. You can also access it remotely through our VPN.
The ideal candidate has knowledge and experience in the community health care setting and is customer service oriented. She/He demonstrates leadership flexibility and creativity in order to provide quality services following best practices in her/his field of expertise. She/He is able to communicate effectively with various health care professionals and physicians.

Summary of duties
Under the direction of the Executive Director and in close collaboration with the Chief of Staff and the Clinical Operations Department the NP will be responsible for:
a) provide primary care clinical services to patients in accordance with best practices and taking into account environmental cultural social psychological and medical factors that influence health within the scope of his/her professional license
b) assess diagnose manage the patient’s therapeutic treatment(s) prescribe controlled substances or cannabis according to the standards set by his/her professional order
c) Collaborate with other health care professionals and service providers to develop a shared vision of care communication strategies and individual responsibilities
d) Refer to other resources as needed to ensure safe competent and integrated care of the client
e) Recognize real potential and perceived conflicts of interest and manage them in accordance with ethical standards
f) terminate the client relationship in accordance with ethical standards
g) Prescribe controlled substances
h) perform controlled acts
i) order and perform certain tests
(j) participate in the development implementation monitoring and evaluation of treatment chronic disease management education counseling and health promotion programs and services
(k) keep abreast of current issues and developments in the field
l) conduct culturally appropriate health promotion activities on relevant topics
m) perform record keeping according to practice standards
n) perform administrative tasks related to clinical functions
o) act as a primary care resource person for the Centre’s employees.
Education: Graduation from a Nurse Practitioner program specializing in primary care recognized by the College of Nurses of Ontario.
License: Member in good standing of the College of Nurses of Ontario (CNO) and an active license (NP)
Language: Excellent command of French and English (oral and written)
Education: CPR ACLS
Skills and experience
Minimum of 2 years of clinical nursing experience as a nurse practitioner with clients of all ages and in a variety of settings.
Experience in prevention education and health promotion in community settings
Excellent interpersonal skills demonstrated ability to work in partnership and establish therapeutic alliances
Excellent organizational skills (prioritize and manage multiple tasks simultaneously)
Autonomy and a good sense of responsibility
Ability to work in an interdisciplinary team in a dynamic and constructive manner
High level of professionalism and judgment
Knowledge of regional and online resources
