Community Engagement Navigator

Location : Remote – Must reside near Ashland or Morehead KY area

This is a field-based position that will serve our members in the Commonwealth with up to 80% of local travel within an assigned area. When you are not in the field you will work remotely from home. Occasional office visits for meetings and/or training are required.

Here’s what a typical day may look like:

  • Responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources.

  • Primary duties may include but are not limited to: Engage with members who have been identified as possessing significant SDOH barriers.

  • Work with the member to establish goals and next steps for addressing SDOH.

  • Support member in selecting provider or programs most appropriate for supporting their needs.

  • Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts.

  • Act as a liaison between referral sources and the health plan.


  • BA/BS degree in Public Health Social Work Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing job development job placement or social work; or any combination of education and experience which would provide an equivalent background.


  • Knowledge of Voc Rehab preferred.

  • Community Health Worker (CHW) certification preferred.

  • Travel may be required.
